File On-Line Go to the Veterans Affairs Canada website - click here VAC Website Then click on the My VAC Account link at the top right: If you don't have an account you will need to register: When you get in to the site click on "Applications and Forms". Then select the form you need and follow the instructions to submit it. Submit a Form Download the application form or go to the Veterans Affairs website Forms at the links below. Then, drop it off at any VAC office, CAF Transition Centre or Service Canada office. You can also mail your completed form directly to the address listed on the form.
